Cardiff and Vale University Health Board are seeking to appoint a Health Disability Activity Practitioner to implement and deliver the Health Disability Activity Partnership across Cardiff and the Vale of Glamorgan.

The partnership is a collaboration between the National Health Disability Activity Team, Disability Sport Wales and local health boards, with the aim of promoting improved health and well-being through increased levels of physical activity. It will support health professionals to successfully advise and signpost disabled people to physical activity (including sport) opportunities in the community, including the delivery of the Health Disability Activity Pathway training, and work with local disabled people to identify and improve access to local physical activity opportunities.

This programme will be supported and led nationally by the Health Disability Team Leader and the post holder will be managed within the physiotherapy service in Cardiff and Vale University Health Board. The post holder will work closely with the health board’s multidisciplinary rehabilitation and health promotion teams, Disability Sport Wales officers and our local community services.

Applicants will have a degree relevant to the role and knowledge of health promotion, Disability Sport Wales and health and social care teams. They will have experience in health promotion type activities, project work and demonstrate strong communication and interpersonal skills. Previous experience of project management, data collection and audit are desirable.
