Inspiring My Journey Award
Nominate up to five people who have inspired your journey in para and inclusive sport.
This award recognises the achievements of the nominator but is intended to celebrate the impact significant people have made to the nominator’s journey to their success.
You can submit your nomination in a number of ways:
- Online via our Nomination Form (below)
- By sending a completed Nomination Form (Microsoft Word document) to awardsnominations@disabilitysportwales.com
- If you would like to submit your nomination as a video (for example if you'd prefer to use British Sign Language), please see Video Submission Guidance

What we’re looking for:
The Nominator (you) will need to have been active within their activity / sport from January 2021 to March 2022.
The Nominator is not the recipient of this award. The Nominator is the individual who primarily has experienced success within their sport or activity, but it is the people that the Nominator identifies (the Nominees) as having played a significant part in their journey and enjoyment of their sport / activity who will be the recipients of this award. Should your application be shortlisted, the nominator may be required to provide further detail to the nomination at the award panel which will take place on 25th April 2022.
The award signifies the importance of a progressive, inclusive, person-centred approach and the continuous, dedicated support and time so many invest in others without being recognised through an awards process.
The Nominator can include up to 5 people who have inspired the nominator to get, be, or stay involved with their activity/sport. The individuals who have inspired the nominator may have been an inspiration in the past or the present and their involvement is NOT restricted to 2021/22.

Video Submission Guidance
If you would prefer to submit your nomination in a video format, for example so that you can use British Sign Language, please make sure to include the same information we require in the written form. You can use the following as a checklist to help you:
- The name of the award you are nominating for (e.g. "Inspiring My Journey Award")
- Your full name
- If you are under 18 years old, please as a Parent or Guardian to join you in the video and say they are happy for you to submit your nomination
- A contact email address, telephone number, and postal address for you, or your Parent or Guardian (if applicable)
- Which club(s) or organisation(s) you are a member of

Details of any awards/recognition received during the your involvement in physical activity / sport
For each of the people who inspired you (max. 5 people):
- The name of the person who inspired you
- The role they held when they inspired you (e.g. Club coach, Development Officer, Parent, Friend)
- Describe how they inspired you
- Their contact details (phone number and/or email address)
Now provide detail about your achievements in the last 12 that have been made possible by the people you are nominating as your inspiration:

This is your opportunity to explain why you feel the nominees identified should be recipients of the ‘Inspiring my Journey’ Award; and how they have contributed to and supported your achievements.

Please discuss the nomination with your nominees and include as much information as possible about their own journey in activity/sport (their achievements, development and strengths), but also the ways in which they have inspired your journey (providing transport, coaching, teaching, cheering-on, officiating at every event, creating accessible sessions, a role model, constant source of positive encouragement, etc).

Submitting your video:
Once you are happy with your video, you can get it to us in a number of ways:
- Send us the video via WhatsApp: +44 7458 031 645. The video will be received by Claire Venn, Disability Sport Wales Business Manager.
- Upload it to a secure Cloud Storage service (e.g. Microsoft OneDrive, Google Drive, Dropbox etc.) and share the file with awardsnominations@disabilitysportwales.com
- Use a large file delivery service (e.g. WeTransfer) to send the video privately to awardsnominations@disabilitysportwales.com
We require names, email addresses, phone numbers etc. to be included in your submission as we may need to contact you as nominator, or the nominee as part of the assessment of the Award. For this reason, your video submission will contain information that should not be publicly shared. We ask that you do not upload your nomination to public video sharing or social media websites and services like YouTube, Facebook, Twitter, Instagram, TikTok etc.
Audio Submission Guidance
If you would prefer to submit your nomination in an audio format, for example if you are more comfortable with spoken language than written language, please make sure to include the same information we require in the written form. You can use the following as a checklist to help you:
